The case for Bad Gastein

  • The Gasteiner Heilstollen is a genuine medical facility: patients are carried into a former gold-mining gallery in the Radhausberg and lie in hot, humid, mildly radon-rich air under a strictly dose-controlled protocol, on referral, principally for ankylosing spondylitis and rheumatic disease.
  • It is the most institutionalised alternative-adjacent therapy in European medicine — in therapeutic use for over seventy years, run in collaboration with the Gastein research institute of the Paracelsus Medical Private University in Salzburg.
  • Austrian, German and South Tyrolean statutory insurers cover the course where it is medically justified. That is a level of institutional seriousness no spa resort can imitate.
  • The valley also has seventeen thermal springs delivering roughly five million litres a day at up to 45.8 °C, so a course here works perfectly well without the galleries at all.

Checkable, not adjectival

  • The Gasteiner Heilstollen runs about 2.5 km into the Radhausberg; therapy conditions are roughly 37–41.5 °C at 70–100% humidity, in mildly radon-rich air. The specific radon concentrations that circulate online could not be substantiated, so we do not publish them.
  • The operator specifies a course of eight to ten gallery sessions of around an hour, with a medical consultation beforehand and supervision throughout.
  • The galleries have been in therapeutic use for more than seventy years, and the site collaborates with the Gastein research institute of Paracelsus Medical Private University, Salzburg.
  • Costs are covered, where medically justified, by Austrian, German and South Tyrolean statutory insurers.
  • Seventeen thermal springs deliver roughly five million litres a day, at temperatures reaching up to 45.8 °C.
The honest caveat. Radon is a Group 1 human carcinogen, and the therapeutic case rests on a contested low-dose hypothesis rather than settled evidence. Older meta-analyses of controlled trials suggested a real analgesic effect; the prospective placebo-controlled RAD-ON02 trial did not confirm it, and the operator itself describes only “a small amount” of radon. The dose is measured and capped, the exposure is medically supervised, and it is excluded in pregnancy and under eighteen. If that balance sits badly with you, take the thermal course in the same valley and skip the galleries — the fortnight works without them, and we will say so rather than sell you the radon as the headline.

The months that actually work

Gastein is a genuine two-season alpine resort. Deep winter pairs the thermal baths with the Ski amadé area; high summer opens the Hohe Tauern hiking that the doctors prescribe as the other half of the cure. April to May and November are the classic Alpine Zwischensaison, when half the valley closes.

90 minutes from Salzburg. The train comes into Bad Gastein station in the middle of the village and the mountain railway is a beautiful hour and a half.
Worth knowing
Take the train from Salzburg or Munich rather than a car. The Tauern line runs into the valley floor, the village is walkable and vertical, and a hire car spends the week in a garage.

The unromantic part

Getting there
Salzburg (SZG) then ninety minutes by train — the line runs into the valley and the station is in the town. Munich is about three hours by rail.
The galleries
Entry is by medical referral, dose-controlled, and excluded during pregnancy and under eighteen. A standard course is around ten sessions.
Insurance
Austrian and German statutory insurers part-reimburse an accredited Kur with a prior referral. Obtain it before travelling.
Language
German. English is spoken at the properties and by the Kurärzte; the paperwork is in German and we translate it.

Questions we are actually asked

Radon. Should I be worried?
It is a fair question and the honest answer depends on your risk tolerance. The exposure is measured, capped and medically supervised, and there is a reasonable older evidence base for symptomatic relief in ankylosing spondylitis — but the best-designed recent trial, RAD-ON02, did not confirm the effect. It is still ionising radiation. If that sits badly, take the thermal and walking course instead; it stands on its own.
Is the winter or the summer course better?
For the galleries it makes no difference — they are inside a mountain at a constant temperature. For everything else, summer, because the graded walking is prescribed and the Hohe Tauern paths are open.
